2. Declaration of consent to act as a director of the company.

3. Confirmation that the individual is not disqualified from being appointed or acting as a director under the Companies Act or any other relevant legislation in Singapore.

4. Signature of the individual consenting to act as a director.

5. Date of signing the form.

This form is an important part of appointing directors for a company in Singapore. Itis typically submitted to the Accounting and Corporate Regulatory Authority (ACRA) along with other relevant documents when registering a new company or making changes to the board of directors of an existing company.
